Martin County EMS confirmed one death and one injury as of 2 p.m. Monday: a husband and wife in the same home south of Shoals.
A tornado warning was issued at 4:30 p.m. and an ambulance was called a little after 5 p.m., Wolf said.
The survivor was flown by helicopter to Ascension St. Vincent Hospital in Evansville, where her condition remains unknown.
Wolf is aware of one other home in Rutherford Township that was struck by the tornado but no other injuries or fatalities.
The glass dome of the historic West Baden Springs Hotel also was damaged during the ensuing hailstorm and its atrium will remain closed indefinitely, although the hotel itself is still open to visitors.
A previous version of this story said there were three touchdowns in the state.
